The Start Menu contains folders (yellow icon) and shortcuts (program icons). This guide will show you how to change the appearance of the icons.
REQUIREMENTS:
1. An icon resource file, for more see here
2. A shortcut (LNK)
Here is a sample icon resource file:

Download the file:
INSTRUCTIONS:
I. Change a Folder?s Icon?
- For instance you?d like to change the icon of ?Accessories? folder?
Type this sample code:
42#\Storage\Application Data\Home\gpc.icons.exe,-8
2. On your sample code, you will notice \Storage\Application Data\Home . This is where you will dump your icon resource file (gpc.icons.exe)
-8 means folder will use icon in red box from ?gpc.icons.exe?
3. Save the sample code as icon.lnk Take note of settings in red box?
4. Transfer ?icon.lnk? to \Storage\windows\Start Menu\Accessories (phone)
5. Close ?appman? using Task Manager (or Xbar), click ?Start? then check if your ?Accessories? icon was changed.
II. Change a Shortcut?s Icon
Assuming you want ?Settings? icon changed?
1. Copy ?Settings? from \Storage\windows\Start Menu to a folder on your PC. Open the LNK in Notepad then edit the code. Always append ?\LOCATION\iconfile.exe,-ID to the code (Thanks to bikiniG for the tip)
For example:
42#:MSSettings?\Storage\Application Data\Home\gpc.icons.exe,-4
Notice that I have added ?\Storage\Application Data\Home\gpc.icons.exe,-4 to the existing code. The code means ?Settings? will use the icon (in red box) of the resource file located at \Storage\Application Data\Home
2. Save the code as Settings.lnk
3. Transfer the LNK file to \Storage\windows\Start Menu
4. Close Start a.k.a. ?appman? for changes to take effect.
5. Click ?Start? then check if icon was changed.
